Terms of Payment

 

Payment is due at the time of service.

 

Balance Medical is honored to be offering the highest quality in men's health at the lowest prices nationwide.

 

Balance Medical accepts cash, check, and credit card, Health Savings Accounts (HSA), Flexible Spending Accounts (FSA) cards. No insurances are accepted at this time. On a case by case basis, some insurance companies do reimburse for Hormone Replacement Therapy and Testosterone Replacement Therapy. Each patient is responsible for contacting their own insurance company to confirm reimbursement. Balance Medical does not contact insurance companies and will not take any responsibility for additional forms an individual's insurance may require. Balance Medical will provide assistance by completing the standard claim form (CMS-1500) for the patient to submit to their insurance company. If a claim form is needed, please email us at contact@balancemedicalok.com  You are responsible for the entire cost of therapy at the time service is rendered.

 

We provide patient therapy as part of an entire therapy package that includes not only your medications, but support from both our administrative and practitioner staff. Therapy is provided on a monthly basis and is continuous. If payment is not received on or before it's due date, a $30 late fee will be applied to the patient's account every 5 days the payment is delinquent. At the end of the 20th day of payment delinquency, late fees will be dropped and the patient will be taken off our list of active patients. A new blood panel and initial exam, at full price, will be required to initiate therapy and become an active patient again.

**Please do not become delinquent in your therapy. Optimal results only come with a dedication to therapy in all it's forms (medication, diet, exercise, lifestyle modifications specific to the individual...). We have had amazing results at Balance Medical and will continue. The percentage of patients with life-changing results that I have witnessed at this clinic are astounding and humbling.
